Town of Bay Harbor Islands Contractor working at 1150 100th Street augured his foundation pile through a sewer lateral; during the concrete pour of the pile, concrete escaped into the broken lateral and caused a blockage from hardened on the 8" sanitary sewer collection main. Sewage vacuumed yesterday to alleviate overflow. 41-5 Produced Water Injection Line Release A release occurred on the 41-5 produced water injection line on February 2, 2018 at 5:42 AM. The St. Regis Control Room Operator noted a high flow alarm and contacted the field operator. The field operator was in close proximity and immediately started the line isolation and called for assistance. The line was isolated by 5:55 AM. Berms were created to contain the released produced water. Vacuum trucks were used to remove the produced water from the release site. The estimated amount of the release is 45 BBLs of produced water. The Injection Line has been depressurized. The Injection Line will be blinded, clamped and flushed with fresh water in preparation for inspection and repairs. FL0040622 South Beaches / 1390 S Patrick Drive 20" ductile iron pipe had a corroded hole in the top that broke loose. Most likely a defect in pipe liner that allowed gases to corrode the iron. About 375,000 gallons was released to the ground and down storm drain that goes to nearby canal. Flow was isolated and stopped by 8:45am. Reverse notification was sent to residents directly affected on canal. Signs posted along S Patrick, canal and residential entrances. Vac trucks collecting what they can. Area will be washed down, limed and roped off. Sampling from canal will occur before removing signs. Wastewater Spill Dania Beach A sewage spill occurred at 5170 SW 40 Avenue in Dania Beach due to damage to a 6" force main caused by a contractor working in the area. The spill amount is estimated to be over 5,000 gallons (pending final calculation). The sewage discharge started at approximately 4:00 pm and was stopped at approximately 7:00 pm. Repairs and clean-up are in progress. 1625 SW 5th Street diesel spill A spill of <25 gallons of diesel fuel occurred from a yacht during a fuel transfer at approximately 1330. The spill volume entered the Tarpon River. Pump Station #3024 Hiawassee Hylands, 1-31-17 A failed isolation valve caused a discharge during the installation of an emergency pump out connection at pump station 3024 Hiawassee Hylands. The valve failure allowed 6000 gallons of untreated wastewater to reverse flow and enter the excavation pit where it was contained and subsequently recovered by Field Services Staff. The excavation pit was located in unpaved land on the south side right-of-way Anoka Drive. No other areas or infrastructure were affected by this event. Field Services staff were able to isolate the force main again and finish the emergency pump out installation. The approximate gallons released and recovered are estimated to be as follows: 6000 gallons released, 6000 gallons recovered, 0 gallons unrecovered, and 0 gallons discharged to surface water. Wastewater Discharge, City of Cooper City, 01-31-2018 Discharge of domestic wastewater at 5101 SW 101 Avenue, Cooper City, FL. Event started at approximately 2:40 PM and discharged approximately 300 gallons into a storm drain inlet that ultimately drains to a storm drain retention pond. Quincy Derailment On 01/30/18 at 0658 hours, Gadsden County Sheriff's Office reported at least four cars derailed at MP SP 823.37 on the CSX Tallahassee Subdivision. This is located at SR 267 (Pat Thomas Pkwy) in Quincy, Florida. JB Train Dispatcher was notified and advised Train with went into emergency and the conductor was walking back. Quincy Fire Department advised they were on scene and reported at least two cars on their side and two cars completely off the tracks. Incident resulted in release of approximately 440 gallons of adipic acid (dry/powder). Materials were removed from land surface by CSX hazmat contractors. Material did not affect soil or groundwater. Release area was neutralized and cleaned with Simple Green solution following removal of bulk product. Minor amount of battery acid was released inside the signal shed. CSX hazmat contractor performed pH testing of the soil surrounding the shed with no detected pH anomalies.
