Pump Station #3216 Padgett Circle, 11-2-18 Pump Station #3216 is currently under rehabilitation by Prime Construction. While construction is underway, flow to this station is being diverted via a temporary bypass pumping system. The bypass pump malfunctioned causing the gravity collection system to surcharge resulting in a discharge from Orange County Utilities manhole 32160015. The discharge flowed down grade over paved road surface and entered a storm water inlet box located along the west right-of-way of Shelburn Court. Approximately 600 gallons of untreated wastewater entered the storm water inlet structure where it was later recovered. Staff that responded to the site immediately prevented the remaining 1400 gallons from entering into the storm inlet structure with the use of a Vactor Truck. No wastewater was observed entering the adjacent canal leading to Lake Jessamine. The responders then washed down all affected areas and applied deodorizer. The approximate gallons released and recovered are estimated to be as follows: 2,000 gallons released, 2,500 gallons recovered, 0 gallons unrecovered, and 0 gallons discharged to surface water. There is little or no potential risk to public health, safety or welfare. 11/02/2018 16:57 EDT OCU Wastewater Pump Station #3216, Padgett Circle 5811 Padgett Circle; Orlando FL; 32839 Orange Eduardo C. Contractor Error Today 11-2-2018 we had a scheduled power outage with our power company. I transferred the plant to generator power and everything was working fine. When the power company came I informed them we already transferred to generator power by pulling the main to our building. In the process of the power company┐s procedures they pulled another main breaker that went to the effluent station without notifying me. When we discovered the problem we also discovered that the safety latch allowing that effluent main to turn station on and off had broken. We were able to get an electrician put to a temporary fix on it and they will be back Monday with correct parts for repair. However, while waiting for the repair we spilled 4,000gallons of treated effluent onto the ground and all was contained onsite. 11/02/2018 16:29 EDT City of Bowling Green 596 Lake Branch Rd; Bowling Green FL; 33834 Hardee Evelyn Guffey Lead Operator evelyn.guffey@hardeecounty.net (863) 205-6181 Operator Of the Facility Evelyn Guffey (863) 205-6181 evelyn.guffey@hardeecounty.net 27.627108 -81.815888 11/02/2018 10:00 EDT No 11/02/2018 14:00 EDT Incident Location

Reclaimed Water Spill at Dixon Blvd. & Pineda St. in Cocoa On October 30, 2018 at 12:30 pm, a contractor for SS Mechanical LLC was pushing a sleeve across Dixon Blvd. at the intersection of Dixon Blvd. and Pineda St. in Cocoa. The contractor hit a 12-inch reclaimed water main and put a hole in it. The City of Cocoa Sewer Field Operations crew shut off valves on the east, west, north and south of the break but still had water coming out and running down the road on Pineda St. The reclaimed water pipe was dug up and the crew put a repair band on the pipe to stop the water from leaking. The pipe repair was finished at 3:00 am the next day, October 31, and restoration was started in the area. The reclaim water was isolated to the area of Pineda St. and Japonica St. The area has no storm structures so the reclaim water soaked into the ground. It's estimated that approximately 80,000 gallons of reclaimed water flowed out of the broken pipe. The State Warning Point Toll Free Number was called on October 31, 2018 at 11:30 am and the incident #2018-6749 was issued. The spill was reported to Department of Environmental Protection's David Smicherko at 11:15 am the same day. For you further assistance, please contact Gary Parker at the City of Cocoa at (321) 433-8755 or by email gparker@cocoafl.org. 11/01/2018 16:35 EDT City of Cocoa Reclaimed Water System Dixon Blvd. & Pineda St.; Cocoa FL; 32922 Brevard Donald Downs Conservation/Public Relations Officer ddowns@cocoafl.org (321) 433-8705 Employee of the owner of the facility.
