The Emergency Contact List has
been updated to reflect the correct number for FDEP and a change to the clinic
name as U.S. HealthWorks has bought Medworks.

Thank you for sending these
verifications that you are doing the appropriate facility inspections and
marking the non-hazardous manifests with proof of halogen measurements. Now all
I need is the corrected emergency contact page (i.e. with the correct FDEP
number) and proof that it has been provided to the local authorities. Thanks
for working with me on this, as I think it is important that we get it all
right now to avoid any issues in the future.

Please see attached copies of 1)
ledger entries with Tank Farm Inspection stamp and 2) non-hazardous waste
manifests with Halogen stamp, showing incorporation of both in normal use.

Looks good, did you receive the
one for the halogen screening yet? I think that is all that is still owed but
don’t take y word for it as I will have to review your submittals. I should be
able to get to that by mid-next week as I have a big fire to put out right now.
Thanks for your cooperation and looking forward to putting this one to rest.
Have a great day!

The new stamp for the tank farm inspections is here, revised to better address all regulatory items. A copy is attached and we are instituting changes to this stamp immediately.

Just a note to let you know I am
still working on these items.
The rubber stamp for the halogen
testing should arrive in about seven days.
The rubber stamp for the
inspection is taking a bit longer.

Steve

I have spoken to Larry about
this, so if you don’t understand my email, talk to him. Your dispatch tickets,
bill of ladings, manifest, whatever you are using for used oil pickups does not
indicate that a halogen screen has been performed. This has to happen. Larry
said you may just get a stamp to show this happening until it is time to
reorder the paperwork you leave for your clients. Please let me know how you
will handle this and send me an example.

Steve, finally got a chance
today to start reviewing your submittal. I am only just beginning but I wanted
to give you a heads up on one thing. Your general facility inspection logs
don’t meet the requirements of 40 CFR Part 279.52(3), as the log doesn’t
include checking you communication or alarm systems on a regular basis, nor
does it include examination of your fire suppression systems, spill control
equipment and decontamination equipment. If you read the rule, it is sort of up
to the facility to decide what is the appropriate frequency to examine these
different items but they need to be checked on some sort of regular basis. What
I would like to see from the facility would be a log that includes these items
that has been filled out for at least a couple weeks or if you intend to make
certain items only monthly checks, then you need to send me the next monthly
inspection and an example log showing what your are inspecting and at what
frequency. I know it is Monday am and I may not be making myself clear so
if you need more interpretation, give me a call. (561) 681-6756 I will
let you know if I find any other issues.
