Transformer Non-PCB Mineral Oil Spill |
Transformer was found laid over on it side. Approximately, 50 gallons of Non-PCB Mineral Oil was spilled on asphalt. Spill was cleaned up and absorbents drummed for pick up for disposal by waste contractor. Transformer was righted and moved to area to be repaired. |

discharges on the ground. Estimated amount of sewage loss was greater than 1000 gallons and likely less than 10000 gallons.
When the problem was identified repair was initiated and completed as soon as possible. |

There was a spill that was caused by a contractor installing fiber. The contractor bored through a force main. The name of the contractor is Vasi and Associates. A 8" force main was broken. About 2,000 gallons spilled. The spill went down a storm drain that goes into a ditch. I walked down and checked the ditch and the ditch was dry. The spill soaked into the ground. The coordinates where the spill occurred is 28.01207 N, 81.64578 W. |

A work order was submitted at 8:02 AM on 3/7 regarding a toilet gurgling in building 29. Our team went out within 20 minutes to check the unit and the drain. The drain line for building 29 was snaked, jetted, and cleaned up. We don't see there being more than 100 gallons of possible spill going into the storm drain if that. A representative arrived this morning to view the location of the incident and our maintenance supervisor did not see any additional residue that would show that another incident happened last night. No additional reports were received at the office regarding an issue in this area. |

Approximately 2,400 gallons of raw wastewater overflowed from a manhole onto the ground and entered a near-by storm drain that feeds a near-by retention pond with no public access. Further investigation confirmed that the overflow was due to grease blockage in a gravity line. The affected area was treated with lime and Operations was unable to recover any of the discharge. The gravity line was also cleaned out to remove the blockage and normal operation resumed. Clean-up actions were completed late in the afternoon of March 7, 2023. |

On 3-7-2023 at 10:20 AM, a garbage truck on normal pick up, experienced a hydraulic hose failure. This resulted in a leak of approximately 45 gallons hydraulic fluid. The location is at main street and 13th avenue. The oil was contained to the roadway and all 45 gallons were recovered. No oil made it to the storm drain. Fleet mechanics were on scene very quickly and used fifteen 20lb bags of dry sweep (kitty litter) to clean up the hydraulic fluid. After the recovery of the dry sweep and oil, the city ran the street sweeper to clean up anything left behind. The hydraulic oil that leaked was Chevron Clarity oil AW68, a heavily refined oil product and as per SDS not classified as hazardous according to 29 CFR 1910.1200 (2012). |

A 24-inch force main split releasing approximately 1,500,000 gallons of raw wastewater onto the ground which flowed into a near-by stormwater retention pond. Due to the volume discharged, a Polk County Utilities (PCU) contracted with an outside agency to assist with clean-up operations. The contractor bypassed the damaged section of pipe using a wet tap until it was replaced, pumped out the storm water pond, and spread lime for disinfection and. The damaged section of pipe was replaced in the early morning hours of March 7, 2023. Fresh water was flushed into the pond and storm drains for additional clean-up and subsequently pumped out. PCU recovered approximately 95% of the amount discharged including any water that was in the pond and properly disposed of it. Signs were also posted around the pond as a precautionary measure while clean-up activities were occurring. All work was completed at 2:30 PM on March 10, 2023. |
