DWU Reclaimed Water DWU was notified about 8:30am today that there was a reclaimed water main leaking at 1001 Airport Road, Unit 17 (Destin Jet). DWU investigated and found a leak at a 90-degree fitting that was encased in concrete. DWU was able to isolate the system by about 11am by closing nearby valves. It is estimated that about 6,500 gallons of reclaimed water escaped the 12 reclaimed water main. No reclaimed water entered any stormwater or wetlands in the area. DWU is currently making repairs and will re-energize the system when completed. No cleanup actions are needed as this is reclaimed water used for irrigation in the area. 01/31/2019 15:39 EST Destin Jet 1001 Airport Road, Unit 17; Directions: Off Airport road in Destin, FL.; Destin FL; 32541 Okaloosa Monica Autrey Operations Manager mautrey@dwuinc.com (850) 699-3665 Operator Of the Facility Lockwood Wernet (850) 200-2028 lwernet@dwuinc.com 30.401643 -86.475874 01/31/2019 08:30 EST No 01/31/2019 11:00 EST Incident Location

SWRF 1030 01/29/2019 The North Plant Aeration Structure, at the South Water Reclamation Facility, overflowed due to a blockage at North clarifier #1. This resulted in a release of secondary treatment MLSS (mixed liquor suspended solids) onto three areas adjacent to the structure. The MLSS was contained in grassy swales and on a non-pervious roadway. Storm water inlets in the area are blocked due to construction. Therefore, no MLSS reached the storm water infrastructure. A Vac Truck was used to recover the MLSS from the grassy swales and roadway. All affected areas were washed down. The approximate gallons released and recovered are estimated to be as follows: 3,000 gallons released, 2,000 gallons recovered, 1,000 gallons unrecovered, and 0 gallons discharged to surface water. There is little or no potential risk to public health, safety or welfare. 01/30/2019 10:02 EST South Water Reclamation Facility (SWRF) 4760 W Sand Lake Road; Orlando FL; 32819 Orange Allison Freese Environmental Engineer Allison.Freese@ocfl.net (407) 836-6944 Environmental Engineer Mark Robinson, Assistant Manager (407) 254-9977 Mark.Robinson@ocfl.net 28.444552 -81.4418 01/29/2019 10:30 EST No 01/29/2019 10:50 EST Incident Location
