MPLX Terminals LLC Tampa Terminal Terminal was performing an EPC approved piping closure by grouting abandoned lines. A small amount of residual gasoline in the line was pressurized by the grouting process and sprayed onto both impervious and pervious surfaces. All material on the impervious surface was collected in the trench drain for routing to an oil/water separator. After vacuum removal of soils from the pervious surface, a sheen was observed on saturated soils in the excavation. An investigation into the source will be initiated. Vacuumed impacted soils will be disposed. 03/08/2018 16:34 EST MPLX Terminals LLC 425 South 20th Street ; Tampa, FL 33605; Tampa  FL; 33605 Hillsborough Robert Jarabeck Sr. HES Professional (440) 465-2365 Environmenatl Engineer for the facility Robert Jarabeck (440) 465-2365 27.959566 -82.370316 03/07/2018 12:30 EST No 03/07/2018 12:40 EST Incident Location

11047 SR 52, Hudson ARV Spill Report called out to a possible F/M break in front of the address 11047 SR 52 in Hudson. Upon arrival Mr. Rau noted that sewage was coming from the structure of below grade ARV. At this time Mr. Rau called out proper personnel to assist with the clean up of spill and repair crew to fix the problem at hand. I (A. Sagastume) arrived on site where Mot and retrieval efforts had already been established. We got the remainder of trucks in place and proceeded to shut down affected area and divert flow so repair could be made. After liquids where drained from the structure we noted that the bands on the 12" x 2" saddle had deteriorated causing the spill. Replaced saddle with a new one and placed system back in normal operation. The spill volume was approx. 8500 gals which we retrieved approx. 6600 gals. Spill did enter a storm collection system. We attempted to gain access to storm system but are no access points on SR 52. Storm collection system is maintained by DOT and flows to retention area approx. 1/4 mile West of spill site on the south side of SR 52. Inspected the pond and did not note any signs of a breach into the retention pond. Used 40 Lbs. of lime and 1500 Gals of H2O to neutralize the impacted area. Spill rate was calculated at Approx. 70 gals a min for a duration of 120 mins. 03/07/2018 20:29 EST Shady Hills Wastewater Treatment Facility 14220 Hays Road; Directions: PERMIT # FLA012741; Spring Hill FL; 34610 Pasco James Hircock Environmental Compliance Specialist (813) 235-6189 Environmental Compliance Specialist James Hircock (727) 247-0391 28.333393 -82.631561 03/07/2018 07:00 EST No 03/07/2018 09:30 EST Incident Location

Overflow into on-site stormwater runoff collection system During a routine outage maintenance and wash down activity being conducted at Crystal River Unit 4 on March 6, 2018, a sump overflow resulted in wash water entering into a pipe trench and then to the site stormwater runoff collection system. The overflow was discovered at approximately 2:30 pm on March 6, 2018, and activities were discontinued upon discovery. It is estimated that approximately between 10,000 to 50,000 gallons of wash water overflowed to the runoff collection system. This runoff collection system has a permitted internal outfall under NPDES permit FL0036366. There was no discharge at this location during the event nor is one expected. Therefore the wash water was contained on-site and no discharge to surface waters of the US occurred. There is no indication that the environment or public health have been endangered. 03/07/2018 14:28 EST Crystal River Units 4 & 5 15760 Power Line Street; Crystal River FL; 34428 Citrus Ilia Balcom Senior Environmental Specialist (813) 362-5123 Senior Environmental Specialist Ana Gibbs (813) 926-7263 28.97 -82.696 03/06/2018 14:30 EST No 03/06/2018 16:30 EST Incident Location
Central Florida Parkway and United Way Force Main, 3/6/18 A four inch PVC force main was found to be leaking untreated wastewater from a split bell end. At this time Field Services staff have excavated the location, reduced wastewater flow and are actively performing repairs at this location. The rate at which the force main is leaking was considered to be quite low at under one gallon per minute. Since the main has now been excavated the flow has increased to approximately 10 gallons per minute. However the event remains under control as the volume of wastewater in being contained inside of the excavation pit where it is continually recovered. Wastewater has not been observed in the nearby canal or drainage swale and any unrecovered volume has been absorbed into the soil. Final repairs will be completed today and at that time total volumes will be calculated and updated. The approximate gallons released and recovered are estimated to be as follows: 2500 Gallons Released, 1500 Gallons Recovered, 1000 Gallons not Recovered and 0 Gallons Discharged to Surface Water. There is little or no potential risk to public health, safety or welfare. 03/07/2018 12:55 EST OCU Wastewater Force Main System ; Directions: Central Florida Parkway and United Way; Orlando FL; 32824 Orange Eduardo C.
