GAF Tampa - Onsite Oil Spill -The GAF Tampa, FL facility manufactures asphalt roofing materials. -Fumes collected during the manufacturing process are routed via ducting to an AirClear filter that is used to reduce emissions. -Condensed oil fumes captured in the AirClear filter are drained to a holding tank. -At approximately 1:11 PM on Sunday March 4, 2018, the plant notified me that the holding tank overflowed into the secondary containment and that a hole in the secondary containment allowed the oily condensate to spill into an on-site drainage ditch. -We estimate the spill to be approximately 200 gallons. -The oil ran down the drainage ditch and made its way to the first of two on-site ponds. GAF deployed spill containment materials to contain the spill within the first pond and has mobilized its spill contractor, Clean Harbors, to assist with cleanup efforts. -The spill is currently buffered from leaving the site by the spill containment efforts, the second pond, and downstream conveyances. As such, there is minimal risk of off-site impacts. 03/05/2018 13:19 EST GAF Tampa 5138 Madison Avenue; Tampa FL; 33619 Hillsborough John Maneth Plant Manager john.maneth@gaf.com (909) 821-0783 Operator Of the Facility John Maneth (909) 821-0783 john.maneth@gaf.com 27.905148 -82.395976 03/04/2018 13:15 EST Yes N Incident Location
Sanitary Sewer Overflow There was a grease blockage in the gravity sewer system which caused a manhole to surcharge. The discharge from the manhole passed through an improperly abandoned private force main directly into a flowing creek. The private force main has now been properly abandoned. Estimated volume of the discharge was 2,000 gallons. Samples were requested. 03/05/2018 12:27 EST Improperly abandoned private force main 4600 Twin Oaks Dr(Ditch in easement at the Northeast corner); Pensacola FL Escambia Robert Gordon Communications Coordinator II scada.operations@ecua.fl.gov (850) 969-6500 Operator Of the Facility Doug Gibson (850) 969-6513 douglas.gibson@ecua.fl.gov 30.411101 -87.279094 03/01/2018 16:00 EST No 03/01/2018 17:00 EST Incident Location
26451 Florida Power Road When I was just leaving one job I checked the alarm screen and seen a dual over run at 1-437 so went to 1-419 station view and found the station in lag and the diversion valve was closed so I headed out to the station. When I arrived the level of the station was high but not in danger of spilling so I started to monitor the station. At about 8:45pm dispatch called me with a high well float at 1-437 so I contacted the second responder and got him in rout. About 15 minutes later the alarm float activated again, so I closed the well lid and went to station 1-437 and raised the alarm float so it would stop calling out, locked up the station and proceeded back to 1-419. I was gone for approximately 7 minutes in total and upon my return I found that the float for the diversion valve had rode the blanket to the surface and had not tilted which had caused the station to spill. I contacted the supervisor on call and informed him of the situation and had him get sludge trucks in route 03/05/2018 09:34 EST Wesley Center Wastewater Treatment Facility 7501 Boyette Road; Directions: PERMIT # FLA016094; Wesley Chapel FL; 33544 Pasco James Hircock Environmental Compliance Specialist jhircock@pascocountyfl.net (813) 235-6189 Environmental Compliance Specialist James Hircock (727) 247-0391 jhircock@pascocountyfl.net 28.200161 -82.376706 03/04/2018 08:45 EST No 03/04/2018 09:30 EST Incident Location
Sunlake Blvd. and Lake Patience Rd. We have a Bypass system that was set up for us by United Rental Services and one of the connections of the bower pipe system had a clamp that came loose and caused Raw Sewage to spill from the piping. We then put cotter pins in all the clamps so this would not come apart again. We also replaced the gasket that torn from the release. We notified all on call personnel and called contractor for pump trucks to assist us with the clean up efforts. In the process of seizing the spill we had sewage come out of multiple locations. M/H in front of Umberland Place and Sunlake Blvd. and the pipe on Sunlake Blvd all of this sewage that spilled on Sunlake Blvd was retained in the retention ditch approximately 1,400 feet in each direction north and South from where the sewage came out of the pipe. A M/H in front of address 19119 Shalott Ct. spilled about 100 gallons in the road and into the storm drain. A M/H in front of address 19048 Tylerton Ct. spilled into the street and into storm drain approximately 500 gallons. 4 M/H's spilled on Hampstead Heath Court in front of addresses 18891,18849 18771, and 18815 all approximately 100 gallons. The address 18891 and 18815 the sewage went into the storm drain. No sewage entered the retention ponds itself however we pulled 6,600 gallons of sewage/ storm water from all storm drains affected which is 2 - 3,300 gallon pump truck loads and spread 10 lbs. of lime around those locations listed above in front of residents homes. The retention ditch that collected the sewage on Sunlake Blvd had 36,000 gallons of Raw Sewage spill into it. 03/04/2018 20:03 EST Land O'Lakes Wastewater Facility 6003 Parkway Boulevard; Directions: PERMIT # FLA012731; Land O'Lakes FL; 34639 Pasco James Hircock Environmental Compliance Specialist jhircock@pascocountyfl.net (813) 235-6189 Environmental Compliance Specialist James Hircock (727) 247-0391 jhircock@pascocountyfl.net 28.222514 -82.501808 03/04/2018 09:00 EST No 03/04/2018 10:00 EST Incident Location
