Mims Water Treatment Plant (PWS 3050834) Bleach Spill Brevard County Utility Services is reporting a sodium hypochlorite spill that occurred at the Mims Water Treatment Plant (PWS 3050834). The spill, which occurred at ~18:30 on 10/13/2020, was originally estimated to be below reportable threshold. However, upon a new sodium hypochlorite delivery to the plant yesterday, 10/22/2020, the amount that was needed to replenish the storage tank was higher than usual. This led operators to revise the estimated release amount from 10/13 to approximately 1,000 gallons, thus triggering the notification process. The spill on 10/13 was due to operator error, as a valve was left open, and the day tank overflowed. Upon discovery of the leak, the valve was shut and the area was flushed with water from a nearby hydrant for approximately 10 minutes. Clean up was complete by 20:15. The spill travelled to a road (Holder Park Dr.) and grassy shoulder adjacent to the plant. No stormwater drains or surface water were impacted. 10/23/2020 12:43 EDT Mims Water Treatment Plant (PWS 3050834) 2262 High Drive; Mims FL; 32754 Brevard Courtney Duff Brevard County Utility Services Dept., Safety and Environmental Coordinator Courtney.Duff@brevardfl.gov (321) 350-8374 Owner Of the Facility Matthew Prendergast (321) 633-2089 matthew.prendergast@brevardfl.gov 28.658954 -80.862925 10/13/2020 18:30 EDT No 10/13/2020 20:15 EDT Incident Location
Miami Everglades Partially Treated Effluent Loss On 10/22/2020 the area manager phoned to report that while doing normal operational visit at 0945 hrs. he found the plant hydraulically overloaded (7" of rain was recorded at the plant) and losing partially treated effluent to the grounds around the clarifier. The plants aeration system was shut down to allow for the solids within the plant to settle. The total estimated volume of partially treated effluent is estimated to be 5000 gallons. The entire contents of the discharge stayed within the perimeter fence. No waterways were affected. The regional manager went to the site and placed a submersible pump in the clarifier level with the effluent weir to assist with transferring the effluent to the Chlorine Contact Chamber when needed. 10/23/2020 10:58 EDT Miami Everglades 20675 SW 162nd Avenue; Miami FL; 33187 Miami-Dade Bobby R Romeo Quality Control bromeo@uswatercorp.net (352) 281-0768 Operator Of the Facility Bobby R Romeo (352) 281-0768 bromeo@uswatercorp.net 25.574951 -80.451646 10/22/2020 09:45 EDT No 10/22/2020 14:30 EDT Incident Location

SSO After a reported depression in the roadway, crews dug it up and found a break in the bottom of the gravity main. A force main was connected just upstream, and when the pump station kicked on it sent a gush of wastewater out of the broken pipe. Repairs needed to be made immediately. During the repairs, crews had to pump out some wastewater from the hole, so a sock was placed on the nearest storm drain prior to pumping. The overflow led to a pond on a golf course. Two samples were collected on 10/22/20 point of discharge from the storm outlet and downstream. Due to the shape and vegetation within the pond, upstream was not applicable. Signs posted. The outlet of the pond leads to Cross Bayou, so spill had potential of reaching Waters of the State.
