18 in Forcemain damage 500 Egret Dr. Contractor ruptured a 18 inch forcemain located just out side of our master lift station located at 500 Egret Dr. We were able to capture the approximate 200000 gallons in a low point of the area effectively making a pond. This pond was bordered by berms to keep the sewage from traveling outward and impacting the surrounding areas. No storm water, canals/waterways, or streets were impacted. Clean will involve the pumping of the captured sewage back into the adjacent force main and is currently ongoing. 08/04/2020 16:13 EDT 18 in Forcemain 500 Egret Dr; Hallandale Beach FL; 33009 Broward Jeff Odoms Assistant Director of Public Works/Utility Operations jodoms@cohb.org (561) 853-8680 Owner Of the Facility Jeff Odoms (561) 853-8680 jodoms@cohb.org 25.981 -80.134 08/03/2020 10:45 EDT No 08/04/2020 05:05 EDT Incident Location

NWRF 17:30, 08-02-20 On 8/2/20 one of the fuses for the control power for the facility lift station failed causing all 3 pumps to be inoperable. The fuses may have failed from a surge in power coming into the treatment facility. The lift station overflowed causing wastewater to flow onto the facility┐s driveway eventually leading to a storm water inlet. The storm water system in this area discharges into an isolated retention pond named "Storm Water Pond A". The storm water pond will be pumped to recover approximately 12,000 gallons. The approximate gallons released and recovered are estimated to be as follows: 2,500 gallons released, 12,000 gallons will be recovered, 0 gallons unrecovered, and 0 gallons discharged to surface water. There is little or no potential risk to public health, safety or welfare. 08/03/2020 15:49 EDT Northwest Water Reclamation Facility (NWRF) 701 W McCormick Road; Apopka FL; 32703 Orange Allison Lucente Engineer II Allison.Lucente@ocfl.net (407) 836-6944 Engineer II Mark Robinson, Assistant Manager, WRD (407) 254-9977 Mark.Robinson@ocfl.net 28.628 -81.524 08/02/2020 17:30 EDT No 08/02/2020 17:45 EDT Incident Location

3027 Woodsong Court, Mulberry Location: Lift Station 23 (3027 Woodsong Court, Mulberry) Start of Incident: 8/2/2020; 1:37pm End of Incident: 8/2/2020; 5:15pm A citizen notified the on-call supervisor at 1:37pm that a manhole was overflowing near 3027 Windsong Court in Imperial Lakes. Upon investigation, it was determined that the lift station auto-dialer did not function properly alerting Operations staff to the high-level alert. Operations staff found that one pump at the lift station was clogged while the second pump was not pumping causing approximately 1,300 gallons of raw sewage to overflow the nearest manhole. At 3:17pm a vacuum truck was called to the station to pump down the volume of the lift station. The discharge had seeped into the ground around the manhole and the area was limed for disinfection. None of the spill entered any surface water bodies nor was any of the spill recovered. The spill did not pose any adverse risk to human health, safety and welfare. Operations staff is investigating why the alarm system malfunctioned. The clean-up action was completed at 5:15pm and the lift station was brought back online. 08/03/2020 12:52 EDT Southwest Region WWTF FLA 012954 2000 4th Street ; Mulberry  FL; 33860 Polk Ashley Pike Environmental Specialist III AshleyPike@polk-county.net (863) 298-4216 Polk County Utilities Jeff Goolsby (863) 344-0249 JeffGoolsby@polk-county.net 27.927 -82.002 08/02/2020 13:35 EDT No 08/02/2020 17:15 EDT N Incident Location
